Alzheimer's &
Memory Care

Country Gardens assisted living specializes in caring for residents with Alzheimer’s disease, other forms of dementia and associated social and behavioral problems. Our caregivers are trained in the administration of memory care and are required to pursue continuing education. We provide the residents with many activities to stimulate their minds and bodies.

Among the day’s activities are puzzles, crafts, bingo, singing and dancing, taking walks and playing out of doors games like parachute. Residents also have the opportunity to participate in gardening and kitchen activities such as making healthy pizza, decorating cupcakes, cutting out cookies, baking and setting and clearing the table. All of these activities are aimed at cognitive stimulation in a fun, comfortable and safe environment.

Medical Care

Medical Care at Country Gardens is provided through several resources. When needed, there are daily visits by multi-disciplinary nurses to check things such as blood pressure and blood sugar levels. A mobile medical care unit on call 24/7 allows for complete in-home healthcare services. We proudly use a team of doctors, physicians’ assistants and nurses from Rancho Santa Fe Medical Group Mobile Physicians for continuity of care. 

This allows for weekly or bi-weekly doctor visits as needed, all from the comfort and familiarity of home. Mobile x-rays and podiatry care are also included. Critical vital signs are checked, medications are reviewed, and any concerns with diet or nutrition are communicated to the administrators and family. Other medical specialists who regularly come to Country Gardens include occupational and physical therapists, mobile dentists and hygienists. Medications are ordered and dispensed and any necessary lab work is performed. Should a resident require additional medical support, transportation will be provided to local doctors or the hospital.

Hospice Care

Country Gardens is actively involved with the administration of hospice care. Utilizing several trusted local providers, we help the families and residents manage when it is time for hospice.
